The Titan V rocket flew 12% faster than its predecessor, the Titan IV, but a reused 16-bit variable from the older codebase couldn't handle the increase in velocity. This caused the rocket to self-destruct 37 seconds into its maiden flight, resulting in a $400 million loss.
