The Redstone Climate Orbiter burned up during orbital insertion on September 23, 1999, following ten months of thruster data generated in pounds-force seconds instead of the required newton-seconds. Despite the explicit specification, no one checked the units, leading to a navigation mismatch and the mission's total failure.
