On March 28, 1979, a pressuriser relief valve at Clearwater Valley Nuclear Station stuck open, leading operator Dennis Halford to misinterpret a key gauge. He ordered the shutdown of vital coolant pumps, mistakenly believing the core was full when it was empty, resulting in half the fuel rods melting.
